Being an engineer that works with ladders and safety cages, I noticed a problem with many ladders in the game. Every ladder with a safety cage is backwards! The cage is supposed to be on the side you climb. If you fall backwards, it hopefully will catch you. The first place I ran into such a cage in the game, I tried to go up the cage side and the game wouldn't let me. Finally I figured out the cage was on the wrong side of the ladder. Whoever modeled the ladder or put them in the game apparently didn't understand the purpose of the cage.

It should be a pretty easy modeling fix. Just make it big enough for Lara to climb through! Right now it appears to be a bit too small.
